Clinical Indications

When to refer (KDIGO 2024 guidelines)

Evidence-based thresholds for specialist nephrology co-management and referral.

Clinical Category Referral Threshold Criteria Specialist Nephrology Rationale
CKD Progression eGFR < 60 mL/min/1.73m² with persistent decline (>3–5 mL/min/year) or eGFR < 30 mL/min/1.73m² (Stage 4) Early specialist intervention to optimize modern renoprotective agents (SGLT2i, finerenone, ACEi/ARB) and manage secondary hyperparathyroidism, anemia, and acidosis.
Albuminuria / Glomerular Stress Urine Albumin-to-Creatinine Ratio (UACR) > 300 mg/g or persistent microscopic hematuria with proteinuria Ruling out glomerulonephritis, initiating disease-modifying therapy, or evaluating the need for ultrasound-guided kidney biopsy.
Resistant & Secondary Hypertension Blood pressure remains >130/80 mmHg despite adherence to 3 full-dose antihypertensives (including a diuretic) Targeted hormonal screening for primary aldosteronism, renal artery stenosis Doppler imaging, and specialized mineralocorticoid titration.
Recurrent Nephrolithiasis Two or more lifetime kidney stone episodes, bilateral stones, or early-onset stone formation (<25 years) 24-hour urine metabolic evaluation (LithoLink) to identify underlying hypercalciuria, hyperuricosuria, or hypocitraturia.
Post-Hospital AKI Follow-up Hospital discharge with acute kidney injury or creatinine remaining elevated above baseline Comprehensive post-discharge outpatient monitoring, safe medication reconciliation, and readmission avoidance.
